If you think that robots, machines, or AI are never wrong, you’re mistaken—sorry to say that. Microsoft’s AI Red Team just released a new whitepaper about the taxonomy of failure modes, which is basically a guide to all the ways agentic AI systems can go wrong.

They grouped failures into:

  • Security issues (like agents being hacked or tricked),

  • Safety issues (like bias or harmful decisions),

  • Within those,Ā new agent-specific problemsĀ andĀ old AI issuesĀ now have higher stakes.

A few eye-openers:

  • Memory poisoning – one email can sneak in a hidden command, and suddenly your AI is forwarding sensitive info to the wrong person.

  • Agent impersonation – a fake agent pretends to be a legit one and hijacks workflows.

  • Bias amplification – agents might favor certain users or actions, often without us noticing.

→ So, how do we fix this?

  • Give agents clear identities and role-based permissions.

  • Control memory access – no blind trust.

  • Build in audit trails and transparency for users.

  • And seriously – don’t trust external data unless it’s verified.

Agentic AI is incredibly promising, but it comes with new risks. This isn’t just about clever prompts or model tuning anymore — it’s about system-level thinking. We need to treat these agents like semi-autonomous coworkers — with responsibilities, boundaries, and oversight.

If we get that right, the future is exciting. If not… well, let’s just say someone’s assistant might accidentally fire an employee one day.
